Please bring the required school, team, camp, or activity physical form with the parent or guardian section completed before arrival. If your child wears glasses or contacts, please bring them.
No. A sports physical is focused on sports and activity clearance. It does not replace a full annual wellness exam with your child’s primary care provider.
If your child has pain, swelling, concussion symptoms, chest pain with exercise, fainting, shortness of breath, or another medical concern, the visit may need to be handled as an urgent care visit instead of only a sports physical. Rocklin Urgent Care has on-site EKG, Testing and X-ray if imaging is needed.
Yes. Please bring the required form from your school, camp, team, or sports organization. The parent or guardian portion should be completed before the visit.

My doctor directed me to Urgent Care as they could not get me in today. I went to the Yrgent Care on Granite Drive in Rocklin. They were great. Renata Holliman is a NP. She cleaned my wound, applied antibiotic cream and bandaged my wound, gave me a tetanus shot, and prescribed an antibiotic. They were quick, professional, and kind. If I ever need to go to Urgent Care again, I will go there hands down. I am 76 years old. My husband is 80 and he fell and hurt himself.
